The Institution at the Center
The Academy of Military Medical Sciences (AMMS) occupies a unique position within China's military medical system. Unlike the PLA General Hospital or one of the military medical universities, AMMS is not primarily a clinical institution. It is the People's Liberation Army's premier biomedical research organization, responsible for advancing military medicine, biotechnology, infectious disease research, pharmaceutical development, genomics, and a broad range of scientific capabilities supporting the PLA.
Though the AMMS does operate two key hospitals, its primary role is not in medical care. Reporting to PLA Central Command, its primary objective is to coordinate and direct PLA research and marshal resources and lead research. Rather than treating patients on a large scale, AMMS develops the scientific knowledge, technologies, and research infrastructure that increasingly underpin China's military medical capabilities.
Over several decades, AMMS has evolved into one of the PLA's principal research organizations in biotechnology and advanced biomedical science. Its researchers have participated in nationally significant work involving vaccines, infectious diseases, precision medicine, regenerative medicine, cell therapies, genomics, and biological defense. These are not isolated military research programs. They occupy many of the same scientific frontiers currently driving global pharmaceutical innovation.
The United States imposed sanctions on AMMS, citing its role in China's Military-Civil Fusion strategy and their activity in biotechnology research. The Bureau of Industry and Security cited the AMMS activity in “... Chinese military end uses and end users, to include purported brain-control weaponry.” The sanctions were not directed at a single research project or individual scientist. They reflected a broader assessment that AMMS serves as a central military institution integrating advanced scientific research with PLA strategic objectives.
If AMMS operated exclusively within a closed military research environment, its role would be comparatively straightforward. One could identify its laboratories, evaluate its research programs, and distinguish them from those of civilian institutions. That is not what the evidence revealed.
Instead, AMMS repeatedly appeared alongside China's leading civilian hospitals, medical universities, State Key Laboratories, biotechnology centers, innovation units, and nationally funded research programs. AMMS scientists held leadership positions within civilian institutions. Its innovation units operated inside major academic medical centers. Its research programs overlapped with hospitals conducting internationally sponsored clinical trials. Rather than existing at the edge of China's civilian biomedical system, AMMS appeared central to it.
The question is no longer whether China's military conducts biomedical research. Nor is the central question whether civilian hospitals collaborate with military researchers. Such collaborations occur in many countries. The more important question is how a sanctioned military research organization came to occupy such a central position within China's broader biomedical research ecosystem, the degree of control a sanctioned PLA organization has over civilian medical institutions and research in China, and foreign involvement with entities effectively disguising the cooperation of a sanctioned entity.
The Reach of the Academy of Military Medical Sciences
If AMMS operated solely through its own laboratories and hospitals, understanding its role would be relatively straightforward. The evidence assembled during this investigation suggests something different. Clinical trial records and institutional databases identify 2,187 medical entities across China with verified military organizational markers, including 1,089 People's Liberation Army units, 303 Military Medical University entities, 90 Theater Command hospitals, and 247 additional military organizations. More importantly, over 1,463 registered clinical trials explicitly identify PLA units, AMMS institutes, or Military Medical Universities as sponsors, co-sponsors, or participating research organizations. Military medicine in China is not confined to a handful of specialized hospitals. It represents a nationwide biomedical research infrastructure embedded throughout China's healthcare and research ecosystem.
The influence of AMMS is not measured solely by the size of its research portfolio. It is reflected in the way its personnel, laboratories, and research programs extend beyond formally military institutions. Database mapping identified 5,481 candidate dual appointments linking military research organizations and civilian medical institutions, producing 6,919 documented hospital-personnel relationships spanning hospitals, universities, laboratories, and research centers. Rather than remaining inside military hospitals, AMMS researchers repeatedly appear directing civilian laboratories, supervising graduate programs, leading nationally funded research initiatives, and managing collaborative research infrastructure.
Senior scientist Wei Yuquan illustrates how these relationships operate in practice. An Academician of the Chinese Academy of Sciences, Wei serves simultaneously as Director of the State Key Laboratory of Biotherapy at Sichuan University's West China Hospital while also leading an AMMS Innovation Unit and serving as Chief Scientist for nationally significant biotechnology initiatives. West China Hospital itself is identified as a sponsor or participating institution in 2,131 registered clinical trials, with more than 412 trials directly involving the State Key Laboratory of Biotherapy. Whether viewed through personnel appointments, laboratory leadership, or nationally designated research programs, the organizational boundary between military and civilian institutions becomes increasingly difficult to distinguish.
The same pattern extends throughout China's broader biomedical research infrastructure. The PLA General Hospital, commonly known as Hospital 301, serves not only as the PLA's flagship medical center but also as one of China's largest clinical research organizations ranking 8th nationally by lead sponsorship. It appears as the primary (lead) sponsor of 447 registered clinical trials, participates in an additional 193 trials as a collaborator, and serves as an active clinical trial site for 1,061 trials. Military medical universities display similar research activity.
The Air Force Medical University appears in approximately 225 registered clinical trials, the Army Medical University in 221, and the Naval Medical University in dozens more. These institutions educate physicians, operate teaching hospitals, supervise graduate research, and participate extensively in nationally sponsored biomedical research. They are simultaneously educational institutions, healthcare providers, and major research organizations.
Military-Civil Fusion provides one of the principal mechanisms through which AMMS extends beyond the formal military medical system. Under China's 13th Five-Year Plan, Military-Civil Fusion was elevated to a national strategy, leading to the establishment of Military-Civil Fusion Innovation Units. Unlike conventional academic collaborations, these Innovation Units operate through formal organizational agreements that include joint personnel appointments, shared state-funded equipment, coordinated laboratory infrastructure, and co-supervised doctoral and post-doctoral training programs. Publicly documented Innovation Units connect AMMS with institutions including Sichuan University, Tsinghua University, and Sun Yat-sen University, creating permanent institutional relationships rather than temporary research collaborations.
The same organizational structure appears within China's clinical trial system. International registries such as ClinicalTrials.gov almost always identify the participating civilian hospital as the clinical trial site. They rarely identify the laboratory infrastructure, innovation unit, or principal investigator directing the underlying scientific work. Our database identified 81,054 hospital entities within China's clinical research ecosystem. Among them, 19,393 exhibit explicit governance or attribution gaps in which the registered trial lists only a civilian hospital while the underlying laboratory infrastructure, principal investigator, biobank, or State Key Laboratory maintains documented ties to military-affiliated organizations. Looking only at the hospital listed in the registry therefore provides an incomplete picture of the institutions participating in the research.
The organizational reach of AMMS extends even further through China's nationally funded scientific priorities. AMMS researchers serve as Chief Scientists and Steering Committee members for major national programs including the National Science and Technology Major Project on Prevention and Treatment of Major Infectious Diseases and the Major New Drug Discovery Program. These initiatives are jointly supported through civilian ministries, including the Ministry of Science and Technology, together with the Central Military Commission Science and Technology Commission. Rather than separating military and civilian research priorities, these programs formally integrate them within a common national scientific framework.
Viewed individually, none of these relationships necessarily appears unusual. Universities collaborate with hospitals. Scientists maintain appointments at multiple institutions. National laboratories cooperate with government agencies. International pharmaceutical companies routinely sponsor multicenter clinical trials. AMMS repeatedly appears at the center of China's most advanced biomedical research, not only through its own institutes and hospitals, but through Innovation Units, State Key Laboratories, dual-appointed scientific leadership, military medical universities, nationally funded research programs, and civilian academic medical centers.
The cumulative evidence suggests that AMMS functions not simply as the PLA's premier biomedical research institute but as one of the principal organizational mechanisms through which military and civilian biomedical research are integrated. Understanding how that integration operates requires moving beyond organizational charts and examining the institutions where those relationships become visible.
rom AMMS to Civilians
The significance of AMMS therefore extends beyond the research conducted inside its own institutes. It sits near the center of a much larger system connecting PLA research organizations with military medical universities, State Key Laboratories, national science programs, civilian academic medical centers, and thousands of individual researchers. The evidence presented here shows several mechanisms through which those connections operate: dual-hatted personnel, Innovation Units embedded within civilian institutions, jointly supported research infrastructure, national programs involving both civilian and military authorities, and clinical research in which the hospital identified publicly may reveal little about the organizations or personnel directing the underlying work. AMMS does not need to own a civilian hospital for the PLA to exercise significant influence over research conducted there.
That distinction matters because AMMS is not simply another government research organization. The United States has sanctioned AMMS and its constituent institutes specifically because of their biotechnology activities in support of Chinese military end uses. Yet sanctions designed around identifiable legal entities encounter a considerably more complicated problem when the sanctioned organization operates through personnel, laboratories, research programs, and institutional arrangements distributed across organizations that remain formally civilian. A foreign pharmaceutical company may never contract directly with AMMS.
Its clinical trial may instead identify a major civilian hospital, university, or laboratory as its Chinese partner. The relevant question is therefore no longer simply whether that hospital appears on a sanctions list. It is whether the researchers, laboratories, infrastructure, intellectual property, and scientific capabilities supported by the foreign company ultimately intersect with a sanctioned military organization. International pharmaceutical research funding and providing data to sanctioned Chinese military groups raises questions.
This creates a difficult problem for governments and multinational pharmaceutical companies. If a sanctioned PLA institution directs an Innovation Unit inside a civilian university, if its scientists simultaneously lead civilian State Key Laboratories, or if civilian research programs share personnel and infrastructure with AMMS, then the conventional distinction between doing business with a sanctioned entity and doing business with an ostensibly civilian institution becomes increasingly difficult to maintain. The issue is not merely one of institutional affiliation. It concerns who directs the research, who controls the laboratory, who benefits from the resulting scientific capabilities, and where those capabilities ultimately reside.
The scale of China's biomedical sector makes those questions impossible to answer through sanctions screening alone. Thousands of hospitals participate in clinical research, hundreds of foreign-sponsored trials operate across China, and major studies routinely involve networks of institutions rather than a single research center. Identifying the name appearing on a contract or clinical trial registration is therefore only the beginning of due diligence.
AMMS provides the clearest starting point for understanding this problem because its military status is not ambiguous. Neither is its sanctioned status. What becomes considerably less clear is where AMMS ends and China's civilian biomedical system begins.
